Care coordination tool — not medical advice

KupaCare is a care coordination and documentation tool. It is not a medical device, emergency service, prescribing tool, diagnostic tool, dose calculator, or substitute for professional medical advice.

Your responsibilities

• You must follow your treating provider's instructions, prescriptions, labels, discharge notes and written care plans.
• In an emergency, contact emergency services or your treating provider immediately. Do not rely on KupaCare for emergency response.
• You are responsible for checking all information entered against the original prescription, discharge note, medicine label or provider instruction.

Early-access limitations

• KupaCare may change, contain bugs, lose care data, or be withdrawn without notice.
• You should not rely on KupaCare as your only source of care information.

Safety acknowledgement

By continuing to use KupaCare, you confirm that you understand these limitations, that KupaCare is for care coordination and documentation only, and that clinical decisions remain with licensed professionals and your care team.
